Risk management is a systematic process used throughout the medical device lifecycle to identify hazards, estimate and evaluate associated risks, implement risk control measures, and monitor the effectiveness of those controls. EN ISO 14971 is the internationally recognized standard that establishes the framework for risk management of medical devices.
Risk management process includes the development of a Risk Management Plan, hazard identification, risk analysis, risk evaluation, implementation of risk controls, assessment of residual risks, and preparation of a Risk Management Report. Documentation generated during these activities forms the Risk Management File, which serves as evidence of compliance with regulatory requirements such as EU MDR / EU IVDR.
In accordance with EN ISO 14971, the process includes:
- Risk Analysis: Identification of hazards and hazardous situations associated with the device.
- Risk Evaluation: Assessment of the severity and probability of identified risks.
- Risk Control: Implementation of measures to eliminate or reduce risks to acceptable levels.
- Production and Post-Production Monitoring: Ongoing evaluation of the effectiveness of risk control measures throughout the device lifecycle.
- Benefit-Risk Analysis: Demonstration that the expected benefits of a device outweigh any residual risks that remain after risk control measures are implemented. Benefits may include therapeutic, diagnostic, or operational advantages, such as improved clinical outcomes, enhanced quality of life, or reduced need for additional medical interventions. The evaluation of benefits versus risks is context-dependent and takes into account factors such as the severity of the medical condition, available treatment alternatives, and user expectations.
Risk management is an essential component of technical documentation. Any residual risks must be communicated to users through labeling, warnings and Instructions for Use (IFU). The risk management process is closely linked to clinical evaluation and post-market surveillance (PMS). Clinical risks identified during clinical evaluation are incorporated into the Risk Management File, while information gathered from PMS activities is used to continuously review and update risk assessments. This ensures that the device's benefits continue to outweigh its risks throughout its intended lifetime.
